It is the patient's responsibility to ensure that we have valid insurance information on file for you. You must present your insurance card at check-in for each visit.
We may ask you to reschedule your appointment, or pay cash for services provided, if you are unable to present your insurance card upon check-in.
Also, be sure to notify us of any secondary insurance you may have, in order to avoid unnecessary bills or delays in services and reimbursements.
