Our staff will ask you to complete a new patient intake form online through our patient portal before an appointment can be made. Once this has been received and your insurance information verified, you will be contacted to schedule your first important. This process greatly streamlines your care with us and reduces delays on the day of your appointment.
Due to limited new patient capacity, it is possible that it may be several months before you may be able to be seen by a physician at our office. Because of this, most new patients can be seen sooner and more efficiently by having an intake visit with a physician assistant provider first.
This intake visit allows for time to thoroughly hear your story and order appropriate lab and imaging studies to be reviewed at your physician consultation visit.
